• All memberships are subject to the approval of SWAN executive committee.
• Subscription fees once paid will not be refunded or adjusted unless the membership is rejected by the SWAN.
• The applicant assumes all the responsibility for the facts and information stated in this membership form.
• The applicant/ Members are liable for prosecution/ expulsion from membership for any misinformation/ discrepancies in the details provided in this form to obtain membership of SWAN.

• By signing this form, the applicant accepts the standard norm of SWAN to let other members use the applicant’s equipment for specific purposes in all usual situations.
• SWAN holds no responsibility for any accidental damage of the instruments in this kind of usage
